A practical academic approach to rhinosinusitis in the emergency department and outpatient clinic, with a dedicated integration of odontogenic sinusitis (ODS). The goal is to distinguish uncomplicated viral rhinosinusitis from true acute bacterial rhinosinusitis (ABRS), identify maxillary sinusitis driven by a dental source that will not resolve with antibiotics alone, detect orbital and intracranial complications early, define the visual difference between benign nasal polyps and dangerous sinonasal masses, and outline the principle of maxillary antral puncture and lavage when indicated.
